Full Proposal
Applicants who have been invited to do so may submit a Full Proposal, giving more information than in the original Letter of Inquiry.
Applications must follow the following format:
Please do NOT submit a Full Proposal unless the ALF has invited you to do so as it will not be considered.
Information on the Organisation
• Full legal name of organisation
• Full street address of organisation
• ABN of organisation
• Is organisation registered for GST?
• Name and all contact details of project representative
• Organisation’s annual budget
• Sources of annual income (e.g., government, fund raising, service fees and others)
• Number of effective full-time staff
• Number of volunteers
• Purpose of organisation

• Brief summary of the organisation’s history
(100 words max)
• Description of organisation’s current programs and accomplishments
(100 words max)
• List of all members of the governing body
Information on the Project for which Funding is Sought
* Description of the project

* Reasons for the need for the project to be implemented

* Explain how the project fits within the organisation’s strategic plan
(50 words max)
* Describe the constituencies that will be served by the project and give numbers of persons to benefit from the project
(30 words max)
* Outline the objectives of the project

* Describe the outcomes of the project for the (medica, scientific and general) communities
(100 words max)
* Describe the methodology that will be used to implement the project
(100 words max)
* Explain if this is a new or continuing project
* Give the timetable for implementation of the project
* Will the project be maintained when philanthropic grants cease and if so how?

* How will the positive impact of the project be sustained?

* Is the project suitable to be replicated in other geographic locations or by other organisations and if so is it likely to be replicated? Give details.
* Give a detailed budget for the project (including salaries, travel, equipment, supplies, evaluation, other)
* Indicate how much you are seeking by way of this proposal and detail your organisation’s contribution to the project, either in kind or in cash
* List other organisations (if any) participating in the project and their in kind or financial contributions
* Explain how success will be measured and how the project will be evaluated

Attachments
A statement that the organisation agrees to the publication in ALF materials of a short report on the project, submitted by the organisation within 30 days of the completion of the project
* Latest Annual Report of the organisation
* Latest audited financial statements
* Certificate from the Australian Taxation Office stating that your organisation is a Deductible Gift Recipient
* Organisation’s Privacy Statement
